Retaining independence
Does the draft strategy provide clear proposals to support older people in their own homes for as long as possible?
The vision of the draft strategy is that, in the future, a smaller proportion of older people will need to move permanently into NHS continuing care facilities or care homes to access the acre and support they need.
We will provide quicker access to community services when they are need, ensuring people use hospital services only when necessary, and improving discharge pathways when leaving hospital. We recognise the need to provide more effective support to carers, and to enable older people to access meaningful and purposeful activities in their daily lives.
To achieve this we will make sure that a range of integrated community-based services are available which support older people to live safely in their communities for longer.
