About the Short Breaks Service
The Short Breaks Service provides breaks for you and your carers. You can have the service in your own home or in other accommodation that meets your needs. The number of short breaks you can have will depend on the sort of needs you have, and the needs of your carer.
You are eligible to be assessed for this service if you have a chronic illness or disability and live alone or with a carer.
What you should know
NHS Lothian is responsible for assessing your health issues and providing rehabilitation.
When will the Council consider you for the Short Breaks Service?
To get this service:
- you must be living alone and have a chronic illness or disability, or living with someone else who would also qualify for the service, or
- you must be living with a carer, or supported by a carer, who needs support and a break from the caring responsibility, or
- you must be living with a carer who has to take a break from caring at short notice, or
- you must be needing a break from your home situation, or
- you must be ready to experience a ‘first taste’ or more independence as part of your care plan.
What sort of service will you get?
Different people have different needs. Your assessor will work out with you what sort of service you need.
