PUBLIC ENTERTAINMENT LICENCE - APPLICATION FOR GRANT/RENEWAL

CIVIC GOVERNMENT (SCOTLAND) ACT 1982
APPLICATION FOR GRANT/RENEWAL OF
PUBLIC ENTERTAINMENT LICENCE
NOTES FOR GUIDANCE

1. Please ensure that all questions are answered.

2. An applicant for Grant/Renewal of Licence shall lodge with the Council along with the completed application form, the application fee of £5.00 non-commercial or £25.00 commercial.

3. The completed application form should be lodged no later than 6 weeks prior to commencement of use of the premises.

4. The applicant should provide to the licensing office the undernoted documentation:-

v Public Liability Insurance of a minimum of £2,000,000.00 for the premises
at least 48 hours prior to any licence being issued.

5. *(a) and *(b) on the application form refers to the Notice.

A Notice is attached.  The Notice must be displayed on or near your premises in a position where it can be read easily for at least 21 days prior to your application.

6. After the Notice has been displayed for 21 days, please return to the licensing office the Certificate of Compliance duly completed with the dates in which the Notice had been displayed at or near the premises.

7. Please return this application form with the appropriate enclosures to:-
