Although most of the forms below can be completed online, some require to be downloaded and printed as they need to be verified by a third party.
Carer
Carer - Provides or receives care or support for another person who resides in the property and is providing care for at least 35 hours per week. This reduction is also dependent on the number of adults residing in the property. Please note that your claim will not be accepted if the person receiving care is cared for by a spouse or partner.
Providing or receiving care
Providing or receiving care - leaves their home to provide or receive care either in a hospital, home or elsewhere, for example supported accommodation.
Long term care
Long term care - a patient having his or her sole or main residence in a hospital, residential care home, nursing home, private hospital or hostel and is receiving care or treatment there.

If you claim a single person discount and a child in your household turns 18 or there is a change in your household, for example someone moves into your home, you must inform the Council Tax team straight away.
