Contacting your GP Practice

On contacting your medical practice you will be asked to describe the nature of your enquiry or concern in a sensitive way.  This allows the patient advisory team to effectively assess your needs and direct you to the most appropriate medical professional within the practice, or refer you onto another primary care team that can best meet your needs.  All information that you give the team will be treated confidentially.

Pharmacotherapy Information

Pharmacotherapy

Each GP practice in East Lothian receives support from Pharmacists and Pharmacy technicians who form the Pharmacotherapy Team.

The Pharmacotherapy Team work within the practice, undertaking a broad range of tasks including:

Supporting patients by:

  • providing direct patient care for patients with chronic conditions such as high blood pressure.
  • reviewing medication for patients on complex and high risk medication regimes for safety and effectiveness.
  • liaising with patients and the local community pharmacists to resolve medicines related queries and short supply issues.

Supporting GP practice teams by:

  • processing hospital discharge and outpatient clinic letters to ensure patients’ medicines are reconciled and prescribed safely.
  • providing advice to healthcare professionals around medicines and medicines governance.
  • supporting changes in prescribing processes such as increasing uptake of serial prescribing for patients on long term medicines.
  • delivering quality improvement initiatives.
  • ensuring medicines prescribed are cost-effective to get more from limited NHS resources.
