The items listed within the booklet are all designed to help with minor tasks inside or outside your home. The self selection service does not affect an individual's right to an assessment. You can request minor adaptations for yourself or on behalf of a relative or someone you care for.
It is important that you read the descriptions carefully as we will rely on the information you provide us to process your order. If you are unsure about any of the items, or you need help completing the form, you can get in touch with us on 01875 824 309. Once completed, it can be posted to the address listed at the top of the form.
If you are requesting an adaptation, we will arrange for a trained and authorised tradesperson to come and install it at a time that suits you. All of the tradespeople we work with will carry identification and will be happy to show it to you before you allow them into your home.
Any requests for equipment and/or adaptations not included in the booklet will require further assessment from a member of the occupational therapy service.
