Requesting support to arrange community activities on your behalf

If you would like help from Social Care Services to arrange a placement at a community service on your behalf, or if you would like to request funding to pay for you attend a service from one of the listed providers, you will need to have a care needs assessment.

We use assessments to make sure our recommendations are appropriate, that they will address your needs and that you meet our eligibility criteria.
