East Lothian Health and Social Care Partnership is committed to supporting people to live longer, healthier lives closer to home, in their own home or in a homely setting.
There are times when you might benefit from getting some additional support to help with daily activities in your own home, and/or engaging in community activities to support your mental wellbeing.
We also can't predict when our circumstances might change. An injury, health diagnosis, or illness deterioriation may mean you require some additional support (short, medium or long-term) to help maintain your independence.
There are a wide range of Social Care support options available including:
- introducing self-managed technology enabled care
- joining / attending community based social activity groups
- getting cooked meals delivered
- making adaptations or home improvements to make the home safer to move around in
- receiving personal care assistance, e.g. dressing and bathing
- or if required, looking at a residential placement which provides enhanced care support to provide respite / a provide a break in caring.
