The Role of the Social Worker

Self-Directed Support applies to children, young people, and their families who have been assessed as needing extra help through Children’s Services.

The social worker will:

  • Listen to the child’s and family’s need and goals
  • Complete an assessment that focuses on strengths and what matters most
  • Work with the child and their family to co‑produce a support plan to identify how outcomes will be met
  • Explain the Self-Directed Support options in a clear and balanced way
  • Review the support received 
  • Social workers follow the national Self-Directed Support Framework of Standards which sets out what good SDS practice should look like.
